What You’ll Do (Impact-Focused Role)
As an LPN, you are a clinical leader and advocate for our residents, supporting both Assisted Living and Memory Care.
You will:
- Conduct resident assessments and develop individualized care plans
- Manage and administer medications and treatments safely and accurately
- Monitor and evaluate resident conditions and adjust care as needed
- Ensure compliance with state regulations and clinical standards
- Provide delegation and oversight to caregiving staff
- Complete required documentation and charting
- Support residents with dignity, compassion, and respect
- Participate in on-call rotation or ensure appropriate clinical coverage
- Complete on-site assessments at least every 45 days
- Provide clinical oversight and task delegation to team members
- Maintain accurate, timely documentation
- Collaborate with interdisciplinary team members to enhance care outcomes
- Support both Memory Care and Assisted Living populations
✔ Active LPN license in Tennessee
✔ Experience in assisted living, long-term care, or memory care preferred
✔ Strong clinical judgment and organizational skills
✔ Passion for working with seniors
✔ Team-oriented mindset with leadership capability
✔ Ability to thrive in a fast-paced, resident-centered environment
