About the Role:
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Charter Senior Living of Chattanooga. As an LPN, you will play a vital role in providing high-quality patient care to our seniors. LPN will be responsible for managing patient care, administering medications, and providing emotional support to our residents.
Responsibilities:
Provide direct patient care to residents, including administering medications, wound care, and treatments
Monitor vital signs, report changes to the healthcare team, and take appropriate action
Develop and implement individualized care plans in collaboration with the healthcare team
Maintain accurate and detailed records of patient care and medication administration
Participate in Quality Assurance Program and adhere to policies and procedures
Collaborate with other healthcare professionals to ensure seamless and efficient care
LPN will also be responsible for performing other duties as assigned, including assisting with activities of daily living (ADLs) and contributing to the overall well-being of our residents.

Requirements:
