Facility: Charlotte Bay Rehab and Care Center
Location: Port Charlotte, Florida
Schedule: Full-Time | 8:00 AM – 4:00 PM
Compensation: $42–$46 per hour (based on experience)

Charlotte Bay Rehab and Care Center is seeking an experienced and motivated Lead MDS Coordinator (RN) to join our leadership team. This role is ideal for a detail-oriented nurse with strong knowledge of the MDS and care-planning process who is passionate about ensuring the highest quality of care for our residents.

What You’ll Do

• Drive Quality Care: Oversee and support the delivery of exceptional long-term care services.

• Conduct Assessments: Perform comprehensive initial interviews and resident evaluations.

• Develop Care Plans: Create individualized care strategies to support short- and long-term goals.

• Lead Collaboration: Facilitate interdisciplinary team meetings with staff, families, and caregivers.

• Engage with Specialists: Partner with providers to enhance resident outcomes.

• Mentor & Educate: Provide guidance to nursing staff and lead in-service training on MDS, RAPS, and Triggers.

• Ensure Compliance: Manage MDS and care-planning schedules to maintain regulatory standards.

What We’re Looking For

• Active Florida RN license in good standing.

• At least 3-5 years of experience in a Skilled Nursing Facility (SNF).

• Strong expertise in MDS, Medicare regulations, and care planning.

• Excellent leadership, communication, and organizational skills.
