POSITION SUMMARY
Provides both intensive case management and outreach to families participating in the Family Self-Sufficiency (FSS) program within Charleston-Kanawha Housing Authority (CKHA). Under the direct supervision of the Chief Operations Officer, the FSS Coordinator is primarily responsible for the management, development, implementation and coordination of case management programs and associated services. The incumbent is accountable for the performance of the case management team, ensuring quality services are delivered according to standard protocols, quality assurance and productivity measures. The position requires the ability to effectively lead staff in all areas of program compliance, addressing all performance issues in a timely manner so as not to compromise access to program services. The FSS Coordinator will produce regular activity reports to document progress, track client satisfaction/outcomes, and reflect the use of best practices for case management services at CKHA. As a liaison to external stakeholders, the position may require attendance and participation at various program related meetings, trainings, etc.
ESSENTIAL FUNCTIONS
The following represents principal responsibilities of the position; however, they are not intended to be all inclusive. CKHA reserves the right to change, reassign, or combine job duties at any time and at its discretion.
Initial Functions
- Directly supervise case specialist staff including ongoing monitoring of work quality, conducting regular performance evaluations, developing performance improvement plans, coaching, mentoring and training of new hires.
- With a focus on meeting overall administrative and program requirements, provide support and assistance to assigned case specialists including providing direct case management services to clients in coordination with and in the absence of the case specialist.
- Research appropriate public and social services agencies, resident groups and private businesses to determine the most appropriate agencies to provide services to family members participating in the FSS program.
- Coordinate the establishment of the Program Coordinating Committee (PCC) by using various resources to determine the appropriate agencies to represent the community in preparing an Action Plan.
- Prepare agendas for meetings; follow up on resources for services.
- Prepare proposals for services needed.
- Draft the appropriate forms and/or letters to 1) recruit representatives for the PCC; 2) notify program participants and residents about the FSS program and request responses to determine interest; 3) track results of recruitment of FSS participants; and, 4) determine eligibility to participant in the FSS program.
- With support of the PCC and CKHA, draft a proposed and final Action Plan.
- Establish procedures for recruitment of FSS participants.
- Establish a system for selecting participants for the FSS program.
- Process applications for participating in the FSS program.
- Determine services needed by participants, and match services and providers with participants.
- Develop the individual training and services plan (ITSP) for the participant.
- Assist participants with tasks associated with enrollment and/or application for the services and programs required in ITSP.
- Negotiate and draft Contract of Participation.
- Develop an orientation for incoming program participants on self-sufficiency expectations and availability of programs.
Ongoing Functions
- Monitor service delivery by service providers.
- Take appropriate action if service delivery is inappropriate.
- Monitor progress of FSS participants including activities completed and services received during the entire period throughout the term of the contract.
- Counsel FSS participants regarding compliance with public housing or Housing Choice Voucher (HCV) program rules and regulations as required.
- Maintain working relationships with service providers.
- Network with various groups/agencies to continue looking for additional resources in the community.
- Update and revise procedures and policies as necessary to remain current with HUD regulations.
- Coordinate efforts and monitor participants who have elected to use the portability of their housing assistance.
- Be responsible for the release of escrow accounts consistent with federal guidelines upon successful completion of the program.
- Provide and/or assist with public presentations regarding the FSS program and its participants to various groups/agencies and other public forums, as requested.
- Coordinate meetings and trainings for FSS participants.
- Prepare annual and monthly reports on the status of the FSS Program.
- Report annually to the participants their account balances, withdrawals, deposits, etc. Maintain accurate FSS status and escrow records for filings with HUD.
- Timely submit/process paperwork for successful FSS participants.
- Coordinate the FSS panel meetings.
- Quality Control all files on FSS participants.
- Investigate and document incidences of fraud or other program violations by participants and recommend if assistance should be terminated. Conduct special reviews if necessary to verify program violations.
- Refer participants to Homeownership Program (HOP) and financial education resources.
- Monitor FSS grant funding opportunities from HUD; prepare and submit necessary documentation.
- Maintain the utmost confidentiality of all clientele information.
- Understand and follow directions.
- Enthusiastically promote the Chief Executive Officer (CEO)’s priorities for the operations of CKHA.
- Keep work area neat and clean in appearance.
- Regular attendance and punctuality are required.
- Perform other duties as assigned.
Requires Bachelor’s degree in social work or a related field. Knowledge of community resource programs; barriers to self-sufficiency; and case management practices and procedures. Requires a valid West Virginia driver’s license.
Must be proficient in Microsoft Operating System as well as Microsoft Office applications (Microsoft Word and Excel). Requires strong skills in organization, concentration, time management, initiative, interviewing, negotiation, customer service, and attention to detail and excellent written and verbal communication. Ability to establish and maintain effective working relationships; support and motivate clients; handle policy and procedural questions; make presentation of programs to individuals and groups; be consistent, honest and trustworthy. Ability to work independently, with minimal supervision. Must be able to deal with frequent interruptions. Ability to successfully work with people of diverse ages, economic and cultural backgrounds.
A minimum of four (4) years’ experience in public, private or non-profit agencies in a position directly delivering social services to low-income clients.
