Position Overview

The Case Coordinator serves as the primary source of information for individuals on their caseload. This role acts as the main point of contact for all external stakeholders, including families, guardians, and regulatory agencies.

Key responsibilities include:

  • Developing a strong working knowledge of all applicable regulations (DDS, MRC, MCB, and internal CRC policies).
  • Maintaining and updating confidential files and electronic records (iCentrix).
  • Observing program rooms and community sites to assess individuals’ strengths, needs, and progress on ISP goals.

 

Key Responsibilities

Program Coordination & Individual Support

  • Develop and maintain positive relationships with external and internal team members.
  • Advocate for individuals’ needs through effective communication.
  • Maintain proactive contact with stakeholders to provide a comprehensive view of an individual’s day at Charles River Center.
  • Use evaluation tools and needs assessments to determine primary focus areas for ISP goals.
  • Collaborate with interdisciplinary teams to develop effective teaching and support strategies.
  • Demonstrate proficiency in Positive Behavioral Supports (PBS).
  • Serve as the primary contact for the Individual’s ISP team.

Documentation & Compliance

  • Develop, update, and maintain all ISP documentation.
  • Ensure timely and accurate record-keeping in iCentrix.
  • Adhere to all mandatory reporting timelines within HCSIS.
  • Accurately report all incidents and restraints.
  • Maintain confidential files and attend ISP meetings.

Onboarding & Staff Training

  • Assist in transitioning new enrollees into the program, gathering necessary paperwork, and preparing case records.
  • Provide guidance to Life Skills Coaches and program staff regarding ISP goals.
  • Train staff in effective implementation of ISP strategies and PBS techniques.
  • Conduct regular observations of individuals in program settings and solicit feedback from Life Skills Coaches.

Program Development & Safety

  • Organize and lead scheduled breakout groups tailored to individuals’ needs.
  • Maintain knowledge of Human Rights, Health & Safety, and Sexuality Policies.
  • Participate in all safety drills and activities, including emergency evacuations and safety protocols.
  • Ensure compliance with health and safety procedures, such as keeping areas clear and knowing fire alarm locations.

 

General Responsibilities

  • Provide support in emergency situations and assist with program coverage when necessary.
  • Participate in job coaching activities as needed.
  • Maintain all required CRC certifications and training.
  • Complete assigned projects to enhance the overall experience of the individuals served.

  • Attend all required meetings and training sessions promptly.
Qualifications:

Education: Bachelor’s degree in human services or related field preferred. Equivalent experience in Human Services may be substituted.

Skills & Experience:

  • Experience with Positive Behavioral Supports is preferred.
  • Proficiency in Microsoft applications.
  • Valid Massachusetts driver’s license and reliable transportation.
  • Must pass a Criminal History Background Check (CORI) and other relevant agency screenings.

Physical Demands:

  • Ability to sit, stand, walk, stoop, kneel, or crouch regularly.
  • Frequent use of hands for handling objects and tools.
  • Ability to lift up to 30 lbs from floor to waist and 10-15 lbs from waist to overhead.
  • Assistance will be provided when lifting individuals.

Language Skills

  • Fluency in English with the ability to read and interpret safety rules, operational instructions, and procedural manuals.
  • Ability to write reports and communicate effectively with individuals, staff, families, and external stakeholders.
  • Capable of presenting information in both individual and small-group settings.
