Responsibilities
- Work with clinical social worker to provide social work services at the service units
- Complete psychosocial assessment for patients as indicated
- Formulate and implement the intervention plan for patients
- Provide concrete case management, follow-up and referral service
- Provide entitlements/benefits eligibility counseling and assist with application
- Assist Clinical Social Workers in providing case management service as needed
- Timely services provided to patients based on the Social Work department guideline
- Complete documentations and reports based on the Social Work department guideline
- Attend case conference, agency meetings and staff meetings
- Participate in community outreach and marketing activities
- Perform other duties as assigned
- Bachelor's Degree in Social Science, Social Work, Psychology, Sociology or other related field
- At least one year of working experience in social services, health care agency or related field
- Good verbal and written communication skills
- Bilingual in English/Chinese (Mandarin/Cantonese) required
- Salary: $50,000.00 - 55,000.00 per year
