Charity Care Group is seeking a Licensed Practical Nurse (LPN) to provide one-on-one skilled nursing care for a 29-year-old male with a G-Tube attending a Suffolk Day Support Program.
This is a daytime weekday position with consistent hours and the opportunity to provide focused patient care in a supportive environment.

- Schedule: Monday – Friday
- Hours: 9:00 AM – 4:00 PM
- Status: PRN (3–5 days per week)
Responsibilities
- Provide skilled nursing care during day support hours
- Manage G-Tube feedings and medication administration according to the care plan
- Monitor patient condition and report any changes
- Maintain accurate nursing documentation
- Work collaboratively with day support staff and family members
- Active LPN license in Virginia
- Current CPR certification
- G-Tube experience preferred
- Experience with individuals with developmental or intellectual disabilities is a plus
