Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong foundation in patient care, demonstrate excellent clinical skills, and have experience working with diverse patient populations, including geriatric, pediatric, and individuals with disabilities. As an LPN, you will play a vital role in providing high-quality nursing services across various settings, ensuring patient safety, comfort, and well-being. Responsibilities
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories using EMR/EHR systems.
- Administer medications and injections accurately, including IV infusions and catheterizations, following established protocols and safety guidelines.
- Assist with diagnostic evaluations such as specimen collection, processing, and medical imaging procedures.
- Provide wound care, including dressing changes and sterile techniques for suturing or catheter care.
- Support patients with activities of daily living (ADLs), including mobility assistance using equipment like Hoyer lifts and managing tube feedings or ventilator support when necessary.
- Document all patient interactions thoroughly in medical records while maintaining strict adherence to HIPAA and clinical confidentiality policies.
- Collaborate with interdisciplinary teams to develop discharge plans, coordinate care for long-term or post-acute patients, and ensure compliance with long-term care regulations.
- Educate patients and their families on health management strategies, medication administration, and disease-specific care such as dementia or Alzheimer's care.
Qualifications
- Valid state licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN).
- Proven experience working in diverse healthcare settings such as hospitals, nursing homes, assisted living facilities, or home care environments.
- Familiarity with EMR/EHR systems such as Cerner, Epic, or Athenahealth for accurate documentation.
- Experience with critical care areas including ICU, emergency room, trauma centers (Level I & II), or medical-surgical units is highly desirable.
- Knowledge of specialized fields such as geriatrics, dementia care (including memory care), hospice care, infectious disease management, and pain management is preferred.
- Proficiency in medical terminology, ICD coding (ICD-9/10), CPT coding, and basic life support (BLS) certification required.
- Strong communication skills with the ability to work effectively with patients of all ages—including pediatrics—and multidisciplinary teams.
- Ability to demonstrate professionalism while adhering to infection control protocols and safety standards in high-pressure environments such as trauma centers or urgent care settings.
