At Chaperone Home Care, we believe care begins at home. We are seeking a dedicated and compassionate Case Manager to join our growing team in Contra Costa County & surrounding Northern California areas.
What Youβll Do:
β Conduct in-home visits and client assessments
β Develop & finalize individualized care plans
β Build trusted relationships with clients and families
β Collaborate with caregivers and scheduling teams
β Earn commission for every client you successfully onboard!
