Job Overview
We are seeking a dedicated and compassionate Case Manager to join our team to operate both male and Female GroupHomes.
The ideal candidate will play a crucial role in supporting individuals and families through various challenges, providing guidance, resources, and advocacy. This position requires a strong understanding of Children and Youth Services, the ability to work collaboratively with clients to develop effective plans that promote their well-being and self-sufficiency.
Responsibilities
- Conduct comprehensive assessments to identify client needs and develop tailored service plans.
- Provide ongoing support and counseling, utilizing techniques such as motivational interviewing and crisis management.
- Facilitate access to community resources including addiction counseling, childcare services, and workforce development programs.
- Collaborate with other professionals in the fields of child welfare, social work, and healthcare to ensure holistic support for clients.
- Monitor client progress and adjust service plans as necessary to meet changing needs.
- Advocate for clients in navigating systems such as independent living skills and
- Maintain accurate documentation of client interactions, progress notes, and case files in compliance with organizational policies.
Experience
- A background in social work or group home experience is a PLUS. Related fields is preferred, with experience in case management or direct client support.
- Familiarity with ADOLECENTS, PA 3800 REGULATIONS. counseling practices and motivational interviewing techniques is highly desirable.
- Experience working within child welfare systems or providing mentoring services is a plus.
- Knowledge of community resources related to childcare, workforce development, and crisis intervention is beneficial.
- Strong interpersonal skills, with the ability to build rapport with diverse populations while maintaining professionalism.
