Reporting to the Chief Program Officer, the Program Manager is responsible for supervising case managers, providing oversight of participant services, and ensuring accurate, timely, and compliant documentation within the organization’s case management system. This role serves as the connective tissue between program design and day-to-day implementation—supporting staff performance, strengthening service delivery, and ensuring that participant data, case notes, and outcomes align with internal standards, funder requirements, and best practices in trauma-informed care.

  • Review case notes, service plans, assessments, and participant records for completeness, quality, and alignment with internal policies and funder requirements.
  • Ensure staff adherence to documentation timelines, data standards, and confidentiality requirements.
  • Collaborate with leadership to improve workflows, reporting processes, and system utilization.
Program Quality & Compliance
  • Monitor program implementation to ensure fidelity to grant requirements, internal policies, and service models.
  • Support program reporting, audits, and monitoring efforts by ensuring documentation readiness and data accuracy.
  • Identify trends, gaps, or risks in service delivery and recommend improvements to enhance participant outcomes.
Cross-Functional Collaboration
  • Serve as a key liaison between frontline staff and senior leadership, elevating operational insights and recommendations.
  • Contribute to program planning, evaluation, and continuous improvement efforts.
Community & Organizational Contribution
  • Participate in team meetings, training, and organizational planning sessions.
  • Support a positive, accountable, and mission-aligned team culture rooted in equity, respect, and transparency.
  • Represent Changing Perceptions professionally in internal and external settings as needed.
