CHANGE, Inc Family Medical Care is hiring Chronic Care Coordinator

Position Details:

· Job Title: Chronic Care Coordinator

· Location: Weirton

· Work Schedule: Monday – Friday

Qualifications:

· Excellent organizational, critical thinking, communication, interpersonal, and multitasking skills

· Strong documentation and computer skills

· Knowledgeable in developing and implementing care plans

· Ability to quickly learn and adapt to new technology

· Ability to work effectively with diverse patients and staff while promoting a positive image for CHANGE, Inc.

· Highly self-motivated with the ability to work independently.

· Current, unrestricted LPN or RN license in West Virginia and Ohio

· Minimum of two (2) years of experience in family and/or internal medicine in an office setting.

Responsibilities:

· Conduct regular telephone outreach to patients with chronic conditions to review provider-established care plans

· Perform medication reconciliation to ensure accuracy and adherence

· Assess barriers to treatment goals and identify opportunities for intervention

· Provide patient education to support disease management and self-care

· Collaborate with Providers and interdisciplinary team members to update and optimize care plans

· Support quality measure gap closure initiatives to improve clinical outcomes

· Collect, analyze, and report data for the Quality Improvement Department

· Assist in performance improvement and quality assurance activities
