LPN Wellness Nurse:
$37-40/hr
Our Chandler House community is looking for a full time or part time LPN Wellness Nurse. We are looking for clinicians who would like to combine their love for people and strong work ethic with the opportunities to advance their career.

RESPONSIBILITIES:
- The primary responsibility of this role includes but is not limited to: monitoring and assessment of resident physical and cognitive functioning; frequent communication with families and external care support teams; preparation, administration and documentation of medications and treatments; submitting and receiving medications from pharmacy; communication with resident physicians and documentation of physician orders.
- Working with care staff to bring about the highest quality of care for residents
Must have patience and understanding, and a desire to work with residents who have a Dementia or Alzheimer's diagnosis. Dependability is critical in this role as well.
