Community Case Manager
Chances & Changes, Inc.
Pay: $20–$24/hour, commensurate with experience
Chances & Changes, Inc. is seeking a compassionate and dependable Community Case Manager to provide direct support and advocacy to individuals and families impacted by domestic violence. The Community Case Manager provides individualized, trauma-informed services to help survivors increase safety, stability, and independence. Responsibilities include case management, safety planning, housing assistance, court support and accompaniment, transportation, crisis intervention, and connection to community resources. The position assists clients with housing searches and applications, benefits and financial resources, court and legal-system navigation, and advocacy with landlords, DSS, schools, law enforcement, healthcare providers, and other community systems. Services may be provided in the office, courthouse, client homes, or other community locations. The Community Case Manager will maintain accurate and confidential documentation, collaborate with agency programs and community partners, participate in required meetings and trainings, and provide direct-service coverage as needed. Qualifications - Bachelor’s degree in Social Work, Human Services, Criminal Justice, Psychology, or related field, or equivalent relevant experience - Case management, domestic violence, housing, court advocacy, or human services experience preferred - Strong communication, organization, advocacy, and problem-solving skills - Ability to work independently and respond effectively to individuals experiencing crisis - Ability to maintain confidentiality and professional boundaries - Valid NYS driver's license and reliable transportation required Required domestic violence advocacy and mandated reporter training must be completed within established timeframes.
