Job Overview
We are seeking a compassionate and dedicated Home Health Aide to provide essential support to patients in their homes. The ideal candidate will play a crucial role in assisting individuals with daily activities, ensuring their comfort and well-being. This position requires a strong commitment to patient care, as well as the ability to work independently and as part of a team.
Duties
- Assist patients with personal care tasks such as bathing, grooming, and dressing.
- Help with mobility and transfers, ensuring patient safety at all times.
- Monitor and record vital signs, including temperature, pulse, and respiration rates.
- Provide companionship and emotional support to patients and their families.
- Administer medications as prescribed and assist with medical equipment usage.
- Maintain accurate medical records and report any changes in patient conditions to healthcare professionals.
- Collaborate with multidisciplinary teams including clinic staff, hospital medicine teams, and behavioral health specialists to ensure comprehensive care.
- Support patients with specific needs such as pediatrics or infectious disease care when applicable.
- Assist with phlebotomy tasks if qualified, ensuring adherence to safety protocols in a Level II Trauma Center environment.
Requirements
- High school diploma or equivalent; certification in home health aide training preferred.
- Previous experience in acute care or hospital settings is an advantage.
- Knowledge of medical records management is beneficial.
- Ability to work effectively in various healthcare environments including clinics, OR settings, and behavioral health facilities.
- Strong interpersonal skills with the ability to communicate effectively with patients, families, and healthcare teams.
- Physical stamina to perform tasks that require standing for long periods and lifting up to 50 pounds when necessary.
- A valid driver’s license may be required for travel between patient locations.
