Responsibilities:
Assess the social, emotional, and psychological needs of residents and develop personalized care plans to address their concerns and improve their overall quality of life
Provide counseling, guidance, and support to residents and their families to help them cope with the challenges of aging or illness
Facilitate communication between residents, families, and healthcare teams to ensure that residents receive holistic care and support
