Champion City Home Health Care is expanding and seeking a motivated PRN RN Case Manager who is interested in growing with a locally owned, community-focused agency.
This role is ideal for an RN who may not be ready for full-time immediately but is seeking an opportunity to transition into a permanent full-time or future leadership role as our census grows.

Responsibilities:
- Complete skilled nursing visits and OASIS assessments
- Assist with Start of Care and recertification visits
- Support case management functions
- Coordinate care with physicians and interdisciplinary team
- Maintain compliant documentation
Qualifications:
- Active Ohio RN license
- Minimum 1 year nursing experience
- Home health and OASIS experience preferred
- Strong organizational and communication skills
- Desire to grow within a developing agency
