What are the responsibilities and job description for the Part-time RN for Telemonitoring position at Champion Care?
Overview:
The Registered Nurse provides exceptional direct telemonitoring patient care to the highest standards of the organization as part of a multidisciplinary team in coordination with the patient's provider and other disciplines for the provision of services, and safe delivery of patient care.
Responsibilities:
· Assesses/reassesses patients’ appropriateness for the RPM program (inpatient, in home)
· Sets up Remote Patient Monitor system in patient's home
· Provides daily monitoring of all patients on Remote Patient Monitor.
· Provides education and instruction to patients/caregivers regarding their ongoing medical maintenance.
· Responsible for completion of all required documentation within 24 hours. This must include any changes in condition and a follow-up with the provider or other disciplines, as appropriate, with the supporting documentation
· Evaluation of patient progress and prompt action if required
· Complies with all Champion Care policies and procedures
· Maintain patient confidentiality while always following HIPAA guidelines
· Remotely monitor patient conditions and stats such as heart rate, blood glucose levels, oxygen levels, etc. and provide follow up to address, report, and resolve any related issues
· Coordinate reporting of data and patient status with Physicians and other relevant medical team members
· Work closely with patient care teams to provide timely intervention and decrease otherwise avoidable hospitalizations
· In non-emergency situations, instruct and educate patients on how to address their care needs (proper medication administration, wound care, diabetes management, and other non-emergency medical help)
· Completing the initial patient assessment to determine home care needs.
· Initiating a plan of care and making revisions as the patient’s status and needs change.
- Coordinating the patient’s care plan and collaborating with other disciplines that are part of the healthcare team.
- Performing appropriate preventive and rehabilitative nursing procedures including wound and ostomy care.
- Educating the patient and family members on following the care plan and medication administration.
- Identifying discharge planning needs as part of the care plan development and implementing prior to discharge of the patient.
Qualifications:
EDUCATION
Nursing Diploma or Associates Degree in Nursing.
EXPERIENCE
Minimum two years nursing experience. Previous home care experience is desirable, but not required.
LICENSURE
Valid Texas Driving License
