What are the responsibilities and job description for the Community Health Worker | Boulder position at CFHW?
As our Community Health Worker, you will work directly in the community with underserved populations to guide and increase community education that would support community members to connect to our services for their behavioral health.
You will work in Boulder County to provide education on mental health facts, available services and create avenue(s) to connect with services. You will be the trusted partner in the community building relationships based on your shared life experiences. You will report to the Community Outreach Manager.
Job Profile:
- Do you have a knack of educating people in a culturally competent presentation, manner, and approach?
- Are you able to connect with people to learn about their barriers and be able to guide them to available resources?
- Are you motivated to reduce health disparities in underserved populations by identifying and understanding barriers they can connect to

What We Need:
- High School Diploma, GED or equivalent
- Must be at least 21 with a safe and clean Colorado driver's license
- Must have reliable transportation to connect with clients in community settings
- Must be connected in the community to connect clients to behavioral health services
- Work some nights and weekends as required
- Lived experience in the behavioral health care system
