In this fast‑paced dual role, you’ll manage a high volume of tasks efficiently while serving as an advocate for patients and the broader community. You’ll facilitate timely referrals when care falls outside of Clinica’s scope—helping ensure underserved individuals are not left without access to essential services. Behind the scenes, you’ll support high‑quality, continuous care by gathering and accurately documenting critical medical information, enabling patients to move forward smoothly on their care journey.

Job Profile:

What You Get to Do:
- You will partner with providers and medical assistants to coordinate referrals that connect underserved patients to essential care such as physical therapy, vision services, and orthopedic specialists.
- You will carefully assess insurance coverage, patient location, and urgency to remove barriers and ensure timely access to care.
- You will send referrals and medical records securely and accurately, so outside providers have what they need to treat patients.
- You will support care teams by promptly prioritizing and responding to referral and records requests, both in person and through instant messaging.
- You will obtain Emergency Room records to help ensure seamless follow‑up care for patients after hospital visits.
- You will keep patient charts complete and up to date by scanning, documenting, and organizing intake forms, referrals, and medical records.
Compensation: Approximately $21.00 - $26.60 per hour. All individual pay rates are calculated based on the candidate's experience and internal equity.
