Responsibilities
- Provide direct patient care, assisting with daily living activities and ensuring comfort.
- Administer medications as prescribed and monitor patients for any side effects or reactions.
- Maintain accurate patient records and documentation using electronic health record systems such as Epic.
- Collaborate with healthcare professionals to develop and implement individualized care plans.
- Support patients with developmental disabilities by fostering independence and promoting well-being.
- Ensure compliance with health and safety regulations while maintaining a clean and safe environment.
- Educate patients and their families on health management, medication usage, and care procedures.
Skills
- Experience in home care or nursing home settings is preferred.
- Strong knowledge of patient care practices and medical terminology.
- Proficient in medication administration techniques.
- Ability to work effectively with individuals with disabilities and developmental disabilities.
- Excellent communication skills to interact compassionately with patients, families, and healthcare teams.
- Strong organizational skills to manage multiple tasks efficiently.
- Familiarity with electronic health record systems, particularly Epic, is a plus.

Pay: $15.75 - $16.20 per hour
