Summary
We are seeking a Licensed Clinical Social Worker to join our team at Candlewood Healthcare & Rehabilitation. In this role, you will provide essential support and guidance to residents, helping them navigate their behavioral health needs while promoting their overall well-being. Your contributions will be vital in creating a nurturing environment for our residents.
This position comes with a starting negotiable salary of $65,000.00 annually
The typical schedule for this opportunity is Monday through Friday from 9am to 5pm. Start and end times are flexible based on community need. This position is a part of the community management team and will take part in the manager on duty weekend coverage rotation.

Responsibilities
- Conduct assessments and develop individualized treatment plans for residents.
- Provide counseling and support to individuals and families dealing with behavioral health issues.
- Collaborate with multidisciplinary teams to ensure comprehensive care for residents.
- Supervise and mentor social work interns or staff as needed.
- Manage program development initiatives that enhance service delivery.
- Maintain accurate documentation of services provided and progress made by residents.
