Summary
As the Social Service Designee at Certus Healthcare Management, you will play a pivotal role in enhancing the quality of life for our residents. In the Social Service Designee position, you will assist residents with discharge planning as well as Social Service needs both while in our care and when they return home. Located in Ohio, this position is integral to our mission of delivering compassionate care while overseeing social service programs that address the diverse needs of our community.
Starting pay for this position, based on experience, is $20 an hour
The typical schedule for this opportunity is Monday through Friday from 9am to 5pm. Start and end times are flexible based on community need.

Responsibilities
- Develop and implement social service programs that meet the needs of residents and their families.
- Supervise and mentor social services staff to ensure high-quality support and care.
- Manage crisis situations effectively, providing guidance and resources as needed.
- Collaborate with healthcare professionals to coordinate comprehensive care plans for residents.
- Ensure compliance with Medicare regulations and other relevant policies.
- Facilitate workforce development initiatives within the social services department.
- Advocate for residents' rights and ensure their voices are heard in all aspects of care.
Salary : $20
