Position Overview
Certified Home Care is seeking a highly organized and detail-oriented Program Coordinator to support the daily operations and compliance of our 245D Home and Community-Based Services (HCBS) programs.
This role is essential to ensuring services are delivered in accordance with Minnesota DHS 245D standards while maintaining accurate documentation, supporting staff, and coordinating with case managers and internal teams.
Compensation reflects an entry-to-mid level range; candidates with advanced 245D compliance and operational experience may find the scope of responsibilities aligns with higher-level roles.
Key Responsibilities
Regulatory Compliance & Documentation
- Maintain compliance with Minnesota 245D licensing standards and DHS requirements
- Ensure all client and employee files are complete, accurate, and audit-ready
- Monitor staff training compliance and documentation requirements
- Assist with preparation for licensing reviews and audits
Program Coordination & Service Oversight
- Coordinate service delivery for:
- Homemaking
- Individualized Home Supports (with and without training)
- Respite services
- Work closely with case managers to ensure service agreements are active and accurate
- Track service authorizations and ensure alignment with services provided
Staff Support & Scheduling
- Assist with staff scheduling and coverage coordination
- Support onboarding processes and ensure training compliance
- Serve as a resource for staff regarding documentation and service expectations
Systems & Documentation Management
- Maintain service documentation and tracking systems
- Utilize platforms such as:
- MN-ITS (billing and claims)
- HHAeXchange (EVV and service tracking)
- Ensure timely and accurate documentation across all services
Billing & Coordination Support
- Verify documentation supports billing submissions
- Communicate with case managers to resolve authorization or service discrepancies
- Support internal processes to ensure timely reimbursement
Communication & Collaboration
- Act as a liaison between staff, leadership, and case managers
- Communicate updates related to compliance, documentation, and service delivery
- Support a team-oriented and client-centered work environment
QualificationsRequired
- Experience working in Minnesota HCBS or 245D programs
- Strong understanding of DHS compliance and documentation requirements
- Excellent organizational and time management skills
- Ability to manage multiple priorities in a fast-paced environment
Preferred
- Experience with MN-ITS and/or HHAeXchange
- Previous coordination or leadership experience in home care or HCBS
- Familiarity with DHS audits and licensing processes
