Responsibilities
- Record a patient’s medical history accurately
- Take and record measurements of blood pressure, temperature, heart rate etc.
- Observe patients under treatment to identify progress, side-effects of medications etc.
- Monitor patients’ condition including fluid intake and output and compose patient charts
- Assist ailing patients in daily necessary activities such as eating, bathing etc.
- Administer injections, prescribed medications, enemas etc., clean and dress wounds and assist with other basic medical care tasks
- Collect samples for lab testing as assigned
- Provide emotional and psychological support when needed
- Communicate with patients’ family or friends to provide advice, comfort and release instructions

Qualifications
- Graduate of an accredited Practical Nursing program or Registered Nurse Program.
- Current, unrestricted LPN/LVN, or RN license in the state of practice.
- Knowledge of long-term care regulations, resident rights, and skilled nursing facility standards.
- Experience providing direct patient care to geriatric and long-term care populations.
- Ability to administer medications, treatments, and physician-ordered therapies safely and accurately.
- Skilled in monitoring residents' conditions and reporting changes to the interdisciplinary care team.
- Experience with electronic health records (EHRs) and clinical documentation.
- Knowledge of infection prevention and control practices.
- Strong assessment, communication, and critical-thinking skills.
- Ability to supervise and delegate tasks to Certified Nursing Assistants (CNAs) in accordance with state regulations.
- Ability to work effectively with residents, families, physicians, and interdisciplinary team members.
- Compassionate, resident-focused approach to care with a commitment to preserving dignity and quality of life.
- Current CPR/BLS certification.
