Pay Rate: $66/hr
Contract: 12 weeks

We are seeking a Physical Therapist for an outpatient rehabilitation assignment in Carlinville, Illinois. This role is ideal for a therapy professional who can evaluate patients, develop treatment plans, and support recovery in a critical access hospital setting.
Responsibilities
Evaluate patients and create individualized physical therapy plans of care
Provide outpatient therapy services to improve mobility, strength, and function
Monitor patient progress and adjust treatment plans as needed
Maintain accurate documentation and follow facility billing procedures
Upload all required documentation according to assignment requirements
Collaborate with providers and rehab team members to support quality outcomes
Requirements
Active Illinois Physical Therapist license required
Minimum 1 year of Physical Therapist experience
Outpatient rehabilitation experience preferred
CPSI/Evident EMR experience preferred
Must review and follow all uploaded assignment requirements
Holiday coverage may be required
No on-call requirement
