Location: At large in Washington County and Centro Cultural Alder 9, 901 SE Morrison St, Portland, OR 97214

POSITION DESCRIPTION
The OHP Assister can help clients navigate the healthcare system by helping those enrolled in the Oregon Health Plan. This position will mainly focus on clients experiencing housing insecurity through Casa Amparo (Washington County) and residents in the Portland area. However, on occasions the OHP Assister may be asked to support and travel to other buildings located in the Metro area. Some of the cases the OHP assister will help with might look as follows:
- Helping clients access primary care, dental services, vision care, and other services.
- Finding providers in the area, educating on Coordinated Care Organizations (CCOs)
- Report changes in the OHP platform
- Renew OHP applications
- Help clients understand the OHP process
- Provide workshops, informational sessions, and tabling
ESSENTIAL RESPONSIBILITIES
- Successful completion of OHP Navigation Specialist training and Certification, or revalidate OHP training.
- Establishing trusting relationships with clients.
- Help clients to enroll in OHP, renew their OHP, change CCOs, and other tasks.
- Facilitate navigation services for medical and non-medical services (social determinants of health) in person, or if needed, over the phone.
- Answer emails and calls to partners and clients in a friendly manner with professionalism.
- Attend virtual and in-person meetings with partners and clients.
- Attend community events, health fairs, and other outreach events may include evenings and weekend events.
- Documents and keeps files of all client services, including date, referrals to other organizations or agencies. Maintain virtual files updated through case notes.
- Compliance with HIPAA.
- Follow written and oral directions and instructions.
- On-site presence required 4 days per week at the discretion of the supervisor.
- Attend mandatory in-person meetings.
- Other duties as assigned

Requirements
- Minimal education required: GED or minimum 2 years working as a Community Health Worker or OHP Assister.
- Complete Background Check
- Ability to work evenings and weekends to meet deadlines
- Possess current driver’s license and auto insurance coverage to commute around the Tri-county, particularly to Alder 9 in Multnomah County (once a week)
- Ability to travel out of town to attend workshops or training
- Willingness to use Salesforce
- Demonstrates customer service skills and proficiency with Google Suite products.
- Learn appropriate questioning content and techniques for interviewing applicants.
- Enter data accurately into an automated system.
- Bilingual in Spanish and English, with the ability to translate documents, agendas, and other materials as needed.
- Maintain and adhere to all Federal, State, and County regulations regarding client confidentiality, specifically 45 CFR-Part 160-164, HIPAA.
PHYSICAL DEMANDS/WORK ENVIRONMENT
This position is approximately 70% in an office environment and 30% in community locations. Office working conditions are standard for an office environment. Community locations include attending and traveling to health fairs and events. While performing the duties of this job, the employee will:
- Engage in regular communication in person as well as through electronic means.
- Routinely use computer laptops, tablets, desk phones, photocopiers, and monitors, including digital screens.
- Frequently lift and carry up to 25 pounds.
- Ability to sit and stand for long periods
- Setting up tables for events, carrying boxes, kneel, bend, crouch, driving or any other physical activity will be needed.
