Private Duty Nurse (RN/LPN) – 1:1 Adult Home Care

Job Summary
Centria Healthcare is hiring compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide 1:1 private duty nursing in patients' homes. You'll deliver skilled, personalized care to pediatric and adult patients in a low-stress, relationship-based setting.
Client-Specific Case Details
This opportunity supports an adult client with a history of a motor vehicle accident (2020) resulting in a C1 ASIA-C spinal cord injury and traumatic brain injury. The client has undergone C1–C2 stabilization and requires specialized, hands-on care.
Care needs include:
- Neurogenic bladder management, including intermittent straight catheterization
- Monitoring for and managing frequent urinary tract infections (UTIs)
- Mobility assistance — client is able to ambulate short distances (up to 40 steps) with a walker
- Support with activities of daily living (ADLs) and ongoing safety supervision
Schedule Needs
Day shifts available: 12 hours per shift, 7 days per week of consistent attendant care.
Responsibilities
- Provide skilled nursing care (trach/vent care, medication administration, catheter care)
- Monitor and document patient condition and vital signs
- Assist with daily living activities (ADLs)
- Communicate with families and care teams Maintain safe, high-quality, patient-centered care
Schedule & Pay
- Full-time, part-time, and per diem shifts available
- Day, night, and weekend shifts
- Weekly pay optional daily pay Competitive pay based on experience

Qualifications
- Active RN or LPN license in the state of Michigan
- Home care or private duty experience preferred (not required)
- Trach/vent experience a plus (training provided) Strong communication and critical thinking skills
