The following essential functions reflect the core requirements of the RN Field Staff Supervisor. Duties may evolve based on client needs, regulatory requirements, or organizational priorities.
Client Visits & Clinical Oversight
- Monitor, schedule, and complete client visits (primarily face-to-face; telehealth as allowed).
- Perform up to 20 visits per week, ensuring compliance with required timelines (Rehab: every 30 days; PDN: every 60 days).
- Prepare and process all documentation associated with scheduled visits.
- Review recent shift notes to understand client status and evaluate documentation quality.
- Conduct root-cause analysis related to client hospitalizations.
- Maintain client care plans, including creating, updating, sending, and tracking POCs and addendums.
- Provide clinical education to clients, families/guardians, and field staff as needed.
- Build relationships with field staff to offer support, coaching, and mentorship.
- Communicate updates on client status promptly and effectively.
- Collaborate with Clinical Leadership, Operations, and Healthcare teams for escalated care needs or staff support.
- Provide a positive and professional experience for all clients and staff.
- Perform Telehealth On-Call responsibilities when required.
- Ensure compliance with Centria’s Code of Conduct, state and federal regulations, and payer/funding source requirements.
- Report any violations of company policies or the Code of Conduct.
- Act as an individual contributor with some financial oversight responsibilities.
- Provide mentorship to designated clinical team members to support skill development and performance.
Education & Licensure
- Current Registered Nurse (RN) license in good standing (required)
- Bachelor of Science in Nursing (BSN) preferred
- Minimum 1 year of nursing experience
- Knowledge and/or experience in home care strongly preferred
- Knowledge of SCI, TBI, respiratory failure, and pediatric populations a plus
- Basic computer proficiency
- Working knowledge of a laptop/desktop PC
- Proficiency in Microsoft Office Suite and Google Workspace (G Suite)
- Ability to follow written and verbal instructions
- Ability to communicate expressively and receptively
- Strong documentation and clinical assessment skills
- Ability to collaborate with interdisciplinary teams (Clinical, Operations, Field Staff)
- Excellent customer service skills
- Highly organized, with the ability to work under tight deadlines and shifting priorities
- Standard office hours: Monday–Friday, 8:30 AM – 6:00 PM
- Occasional schedule adjustments or additional time may be required to meet company or client needs.
- Occasional bending, reaching, lifting, pushing, or pulling up to 30 lbs
- Prolonged periods of sitting, along with intermittent walking or standing
- Frequent use of computer screens requiring visual acuity for data review and documentation
