Centre HomeCare Inc is a trusted leader in home health and hospice care, and we're looking for a compassionate, skilled Registered Occupational Therapist (OTR) (full-time or part-time) to join our team.

In this role, you'll bring expert care directly to patients in their own homes across Centre and sourrounding counties— helping them regain independence, rebuild strength, and improve their everyday quality of life.

What You'll Do:

• Evaluate patients' functional abilities and identify areas for improvement
• Design and implement individualized treatment plans tailored to each patient's goals
• Deliver therapies that address physical, cognitive, and psychological challenges
• Track patient progress and prepare detailed clinical reports
• Collaborate with our care team to ensure a holistic approach to patient wellness

What We're Looking For:

• Active Registered Occupational Therapist (OTR) licensure
• At least one year of experience in a hospital, long-term care, or home health setting preferred
• Strong communication skills and a patient-centered mindset
• Ability to work independently and manage a flexible schedule
