Conducts intake and preliminary interviews to determine needs of clients/families.
Provides support to assigned group of individuals.
Conducts counseling sessions as needed.
Investigates allegations, provides advocacy, and assesses degree of risk to client.
Develops and implements case plans for client based on needs.
Provides for necessary arrangements according to client's needs; contacts other community services/agencies as necessary.
Arranges transportation for clients' appointments.
Testifies in court if necessary regarding clients' needs.
Maintains accurate documentation and records on clients and their progress.
Prepares required reports as mandated by federal, state, or County policies and procedures.
Makes home visits to clients as necessary.
Performs emergency on-call duty.
Other duties as assigned.
