Who We're Seeking:

We're seeking a Licensed Practical Nurse (LPN) to join our Home Health teams. Your role involves monitoring and reporting on patients' medical and emotional well-being. Additionally, you'll communicate with the overseeing RN to coordinate support from the home health team. This position focuses on delivering care to individuals in their homes.

Your Responsibilities:

• Determine patient care needs and develop goal-oriented care plans in consultation management.
• Ensure accurate and timely documentation of patient information, adhering to HIPAA regulations.
• Work autonomously under RN direction and utilize supervisory support when necessary.
• Empower clients to explore care options and alternative plans to achieve desired outcomes.
• Participate in on-call rotation and respond to calls promptly.

Requirements:

• Valid Vermont LPN License
• Minimum of one year progressive clinical experience. Home health and wound care experience preferred.
• Valid Vermont Driver’s License
