Education and Experience:
- Must have bachelor's degree in social, behavior or human services field from an accredited university
- Must be able to qualify as a Qualified Mental Health Professional (QMHP)
- Must have valid TX driver's license and be insurable
Job Purpose:
- Ensures a person is provided uninterrupted services during a transition between inpatient and outpatient services
- Assists with admissions, discharges, and tracking of significant events
- Participating in developing and reviewing a person's recovery or treatment plan
Job Function:
- Work with Intake LPC and treatment team to provide any valuable information for on-going assessments
- Maintain contact with hospital staff for all consumers in the hospital
- Coordinate discharge and follow-up services including doctor appointments
- Provide crisis intervention services as needed
