What are the responsibilities and job description for the Social Worker position at Central Peninsula Hospital?
POSITION DESCRIPTION: Assures continuity of care during the transition from the hospital to post-hospital care by assisting patients and their families with personal and environmental difficulties as they transition from CPH to home or other health care providers.
REQUIREMENTS:
- Bachelor Degree in Social Work, MSW preferred
- State of AK Social Worker Licensure or eligibility to obtain upon hire
- BLSP certification with in 30 days of hire
