Key Responsibilities

• Provide comprehensive wound care, including assessment, treatment, and documentation.
• Perform various treatments and procedures as required by patient care plans.
• Utilize Sigma Care software efficiently for documentation and care coordination.
• Maintain organized and detailed patient records to ensure continuity of care.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans.
• Demonstrate flexibility in scheduling and adapting to changing patient needs.
• Deliver friendly, empathetic, and people-centered care to enhance patient experience.

Qualifications

• Current Registered Nurse (RN) license in the state of New York.
• Current CPR
• Proven experience in long-term care settings.
• Strong skills in medication administration and wound care.
• Proficiency in Sigma Care or similar electronic health record systems.
• Excellent organizational and time management abilities.
• Flexible and adaptable to changing work environments and schedules.
