What are the responsibilities and job description for the Transitional Care Admin (Cape Coral) position at Central Florida Inpatient Medicine?
Job purpose
The Transitional Care Admin/Coordinator is responsible for educating patients about our Home Visit program and scheduling appointments to avoid re-hospitalizations. This position cold-calls patients, therefore having a sales-mindset is ideal! This is an in-person position, located at our M&H Housecalls office in Cape Coral FL.
Duties and responsibilities
- Outbound calls to patients to introduce our Home Visit Program
- Scheduling patients by geographical area
- Using proprietary computer database to enter and assign patients to providers
- Submitting paperwork to Primary Care Physicians
- Making follow-up calls to patients post-appointment(s) to identify further needs
- Communication to referral sources in the community
Qualifications
- Minimum 1 year of experience scheduling patient appointments
- Experience with sales a plus!
- Minimum 1 year of experience in a healthcare setting
- High School Diploma or GED
- Self-motivated in remote setting; ability to be productive, free of distractions, in a 'home-office' environment (orientations, trainings, meetings may require travel from time to time)
- Utilization of many computer software systems, our platform, hospital systems, Microsoft etc.
- Ability to multi-task
- Bi-Lingual (English & Spanish) preferred not required
- Excellent written communication skills
- Must be comfortable being on the phone for several hours a day
- Must be friendly & courteous and have patience on the phone, as we are very often assisting an elderly demographic
- Compensation negotiable based on experience
