.We are seeking a dedicated and compassionate Nurse Practitioner to join our healthcare team. This role involves providing high-quality patient care, performing assessments, and developing treatment plans to improve patient outcomes. The Nurse Practitioner will work collaboratively with physicians and other healthcare professionals to ensure comprehensive care for patients.
Duties
- Conduct thorough patient assessments and evaluations to determine health status.
- Develop and implement individualized treatment plans based on patient needs.
- Perform procedures such as venipuncture and infection control measures.
- Provide urgent care services as needed, ensuring timely intervention for acute conditions.
- Collaborate with interdisciplinary teams to coordinate patient care, including memory care and senior care services.
- Utilize electronic health record systems such as Cerner for documentation and patient management.
- Educate patients and their families about health management, preventive care, and disease processes.
- Monitor patient progress and adjust treatment plans as necessary.
- Maintain compliance with all healthcare regulations and standards.
Qualifications
- Graduate Nursing or related field from an accredited program.
- Current Nurse Practitioner license in the state of practice.
- Experience in gastroenterology, geriatrics, or internal medicine is preferred.
- Proficiency in performing venipuncture.
- Strong knowledge of infection control practices within a clinical setting.
- Excellent communication skills with the ability to build rapport with patients and families.
