The Central Connecticut Coast Y's Alpha Community Services branch is seeking a compassionate and driven Case Manager to support our Family Stability Program. This role supports individuals and families experiencing homelessness or housing insecurity, providing case management services that promote long-term stability and self-sufficiency. As a Case Manager, you will work collaboratively with clients, community partners, and internal teams to connect individuals with essential resources, develop personalized support plans, and advocate for the well-being of those we serve. This position is a full-time, non-exempt role, primarily working Monday through Friday, with occasional Saturdays as needed.
ESSENTIAL FUNCTIONS:
- Models the YMCA core values of caring, honesty, respect, and responsibility in all interactions with clients, colleagues, community partners, and vendors.
- Conducts intakes, assessments, and ongoing reassessments; identifies barriers to housing stability and prioritizes needs in partnership with the client.
- Develops, implements, and updates individualized service plans using person-centered and strengths-based practices; supports clients in setting goals and tracking progress.
- Provides housing-focused case management, including housing navigation, linkage to community resources/benefits, tenant education, and support with documentation and appointments.
- Builds and maintains relationships with community providers, landlords, and property managers; coordinates referrals and, as appropriate, supports problem-solving/mediation to sustain housing placements.
- Provides consistent outreach and follow-up, including communication with clients who are difficult to reach; maintains professional boundaries and responds appropriately to crises and urgent needs.
- Maintains accurate, timely, and confidential client records; completes required documentation, data entry (including HMIS and other required systems), and outcome reporting in accordance with funder and regulatory requirements (e.g., HUD/DMHAS/DOH, as applicable).
- Participates in program monitoring, quality assurance, audits, and case conferencing as required; implements corrective actions and process improvements in coordination with leadership.
- Knows, follows, and enforces Association policies, confidentiality requirements, and all safety/emergency procedures, including those related to mandated reporting and the prevention of child abuse.
