Location and Commitments
This position is based in Monterey, CA, serving the Central Coast region through a mix of fieldwork, outreach, and operational coordination.
This is a part-time role with flexible scheduling with the potential of becoming full-time. Shifts may be assigned between 6:00 AM and 8:00 PM, Monday through Sunday.
Qualifications
Minimum Qualifications
- Bilingual in Spanish and English.
- High school diploma or equivalent.
- Valid driver’s license, reliable transportation, and proof of insurance.
- Ability to pass background checks required for working with vulnerable populations.
- Strong communication and teamwork skills.
- Commitment to upholding dignity, respect, and confidentiality in client interactions.
Preferred Qualifications
- Lived experience with social issues similar to those faced by our clients (such as substance use, homelessness, or justice involvement).
- Possess a state-approved PSS, CHW/P/R Certificate, or meet the requirements for the Certified Community Health Worker (CCHW) credential.
Responsibilities
- Safely drive and operate a work truck to outreach events and community sites.
- Participate in Substance Use Response Team (SURT) shifts, engaging with the public and distributing basic needs supplies.
- Conduct Enhanced Case Management (ECM) services, including: managing a client caseload; conducting client assessments and home or community visits; developing and updating individualized care plans; coordinating referrals and linkages to medical, behavioral health, and social services; and completing timely documentation, notes, and follow-up.
- Maintain a clean, organized, and stocked work truck and workplace.
- Attend partnership meetings and community events as a representative of CCODP.
- Perform other related duties as assigned to support client and community health.
