CASE MANGER
Central Arizona Aging is accepting applications for one (1) Full-Time Case Manager to assess client needs for in-home services. Job duties include screening clients for appropriateness for the program; assessing the functioning, conditions and needs of clients; clarifying client expectations and working with clients to develop individual service/care plans; arranging and ensuring that services and other help are provided and utilized effectively; periodically updating knowledge of the client and monitoring their situation to revise individual service/care plan as needed; managing a case load of clients and providing case management in a specific geographic (service) area and/or for a specific program. Agency vehicle assigned for required travel.
BA/Social Work related field or 4 years experience in social or health services, or combination of education/training/experience which indicates ability to perform job duties. Microsoft Word/Excel. Skill to interview clients and prospective clients. Communicate effectively verbally and in writing and the ability to follow written and verbal instructions. Current AZ Driver’s License with 36 month driving report. Current Level 1 Fingerprint Clearance card and pass Central Registry background check.
