Overview

Central Arizona Aging is seeking a dedicated and compassionate Case Manager to join our team. This position performs case management activities of moderate difficulty for routine to difficult clients, following general case management standards and practice guidelines which include: screening clients for appropriateness for the program/eligibility; assessing the functioning, conditions and needs of clients; clarifying client expectations and working with clients to develop individual service/care plans; arranging and ensuring that services and other help are provided and utilized effectively.

Duties & Responsibilities

  • Perform case management activities including: intake, initial eligibility assessment and on-going assessment, service/care plan development and implementation, service/care plan monitoring, reassessments and service/plan closures.
  • Authorize and monitor services provided.
  • Maintain client records in compliance with contract, CAA, State and Federal requirements.
  • Prepare reports and statistical data as assigned to ensure compliance with contract, CAA, State and Federal requirements.
  • Provide case management at the request of and with informed consent of the clients, while striving to honor the values and preferences of the client.
  • Complete a comprehensive assessment, using approved assessment tools and following contracted guidelines and CAA policy, of the client's functional and cognitive capacity and limitations, needs, strengths, abilities, and resources. Synthesize this information to determine the client's need for services, support, and resources.
  • Develop and individualized service/care plan, based on the assessment, which reflects the client's values and preferences, including a list of problem-oriented goals and services (both formal/informal and/or paid/unpaid) used to achieve the identified mutually agreed upon goals.
  • Case conference with providers to assure that client needs are being met according to identified goals.
  • Maintain accurate and timely documentation of case notes and activities as identified in contract, CAA policies, and State and Federal regulations.
  • Participate in CAA staff meetings and as assigned other agency meetings. As assigned, participate in community outreach activities, continuing education opportunities and other activities as determined appropriate to benefit the service.

Minimum Qualifications

  • A bachelor's degree from an accredited college in a field related to case management such as social work, nursing, public health or other human services related field. Comparable professional knowledge, skills, and abilities that documents 4 years of experience specific to case management may be substituted for the degree. Case management training may include psychosocial assessment of clients; interdisciplinary care coordination; monitoring of health and social service delivery to maximize efficiency/cost-effectiveness; knowledge of resources available to target populations; development and utilization of client-centered care plans; data privacy and confidentiality.

Knowledge, Skills and Abilities

  • Knowledge of case management principles and practices. Skill to interview clients and prospective clients. Ability to establish and maintain effective working relationships with employees, other agencies and the public; to communicate effectively verbally and in writing and the ability to follow written and verbal instructions. Knowledge and application of Basic English, grammar, spelling and composition, bi-lingual (Spanish) is a plus. Ability to type and knowledge of Microsoft Office. Must have Level 1 Fingerprint Clearance Card and able to pass Central Registry background check, 36-month clean driving record. Ability to work with people of various age groups and backgrounds.

Pay: $20.03 per hour
