Travel will be necessary, the case load will be split between our Clanton and Pelham locations.
High Intensity Care Coordinator
Classification: Intensive Care Coordinator
Minimum Qualifications: Bachelors Degree, preferably in a Social Science field, a valid Alabama Drivers License, Maintenance of a good driving record, Personal Automobile Liability coverage ($20,000 - $40,000), and completion of Department of Mental Health approved Intensive Care Coordination training.
Primary Job Functions & Performance Expectations
1. Provide Care Coordination services to individuals with serious emotional disturbance (SED):
- Serve as a liaison with community agencies (e.g., local hospitals, DHR) to identify and coordinate care for SED referrals.
- Make initial client contact within one week of referral.
- Develop and implement individualized service and safety plans addressing clients’ specific needs.
- Engage appropriate support agencies in service planning and delivery.
- Deliver direct case management services, including service linkage, crisis intervention, arranging services, advocacy, needs assessment, home-based services, and transportation coordination.
- Conduct regular progress assessments and update treatment plans per DMH Standards.
- Complete all statistical, administrative, and clinical documentation within one business day of service delivery.
- Ensure documentation is clear, accurate, and compliant with Center policies and DMH Standards.
- Use sound judgment in making referrals to external services.
- Meet quarterly productivity targets.
- Provide accurate consultation and maintain effective communication with community partners.
2. Respond to crises and ensure client safety:
- Provide crisis intervention and emergency response as needed, following Emergency and Safety Policies and Procedures.
- Ensure clients’ safety during transport to the Center or other appointments.
3. Serve as a resource and advocate for mental health services:
- Deliver professional, audience-appropriate presentations to the community, partner agencies, and the general public.
- Promote mental health awareness and advocate effectively for services within the region.
4. Maintain and enhance professional skills:
- Actively participate in workshops, in-service trainings, and Quality Assurance reviews.
- Stay current by reading professional literature.
- Seek and utilize consultation from colleagues when appropriate.
5. Collaborate effectively with support staff:
- Provide timely, clear, and accurate information to support personnel.
- Foster positive working relationships across teams.
6. Perform other duties as assigned by the supervisor:
- Complete additional responsibilities with accuracy, timeliness, and respect for confidentiality.
- Participate in at least two Consultation and Education activities per year.
