About the Position:
We are seeking a highly motivated and dedicated Physical Therapist Assistant to join our team at CenterWell Home Health. As a member of our comprehensive home health care team, you will play a vital role in helping patients achieve their rehabilitation goals and regain their independence.
Responsibilities and Requirements:
- Administer physical therapy treatment plans as developed by the Physical Therapist.
- Document patient progress and outcomes accurately and in a timely manner.
- Participate in ongoing education and training to stay current on best practices and industry developments.
- Maintain open and effective communication with patients, families, and caregivers to ensure informed decision-making and optimal care.
- Meet the minimum requirements listed below:
- Associate degree in Physical Therapy from an American Physical Therapy Association.
- Current state license to practice Physical Therapy.
- Valid driver's license, auto insurance, and reliable transportation.
- Proof of current CPR certification.
- Minimum two years experience as a Physical Therapy Assistant.
- Comprehensive knowledge of the physical, emotional, social, and biological changes that occur in the geriatric population.
- Self-directed, enthusiastic, and accepts constructive feedback.
- Demonstrates good verbal and written communication and organization skills.
- Communicates and demonstrates a professional image/attitude for patients, families, clients, coworkers, and others.
- Maintains current licensure certifications and meets mandatory continuing education requirements.
- Must read, write, and speak fluent English.
- Must have good and regular attendance.
