About the Role:
We are seeking a highly skilled and compassionate Home Health RN Case Manager to join our team at CenterWell Home Health. As a key member of our care team, you will be responsible for providing personalized home healthcare services to patients managing chronic conditions or recovering from injury, illness, surgery, or hospitalization.
Your Key Responsibilities Will Include:
- Providing admission, case management, and follow-up skilled nursing visits for home health patients.
- Administering ongoing care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager.
- Conferencing with physicians in developing the initial plan of treatment based on physician's orders and initial patient assessment.
- Implementing patient care plans in conjunction with patients and families to assist them in achieving optimal resolution of needs/problems.
- Coodinating/overseeing/supervising the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers.
Requirements:
- Diploma, Associate, or Bachelor Degree in Nursing.
- A minimum of one year of nursing experience preferred.
- Strong med surg, ICU, ER, acute experience.
- Home Health experience is a plus.
- Current and unrestricted Registered Nurse licensure.
