Bishop Rehabilitation & Nursing Center is seeking a Social Worker to work part-time for skilled nursing home located in Syracuse, NY. The ideal will have previous healthcare experience!
Duties Include:

Known for its commitment to compassionate care and clinical excellence, Bishop offers both short-term rehabilitation and long-term skilled nursing services in a supportive and homelike environment. Our dedicated team uses a personalized, multi-disciplinary approach to help residents recover, regain independence, and thrive.
Duties Include:
- Maintaining and/or improving the resident’s ability to control everyday physical needs and mental/psychosocial needs
- Identifying individual social and emotional needs
- Developing and maintaining individualized care plans
- Participating in the planning of the resident’s admission, community re-entry, or transfer to another facility
- Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
- LMSW Preferred
- Previous Long-Term Care/Sub-Acute Experience Required

Salary : $50,000 - $60,000
