Bishop Rehabilitation & Nursing Center is looking for a RN Admissions Nurse to work in our facility in Syracuse, NY.

Duties & Responsibilities:
- Complete admission assessments for new residents and hospital returns, including starting the initial plan of care.
- Help support and guide the Nursing Department as directed by the Director of Nursing.
- Review PRI information and work with the interdisciplinary team to help coordinate the admission process.
- Review resident records and complete audits to make sure facility standards and policies are being followed.
- Review and update resident care plans when there are changes in the resident’s condition.
- Assist with discharge planning as needed.
- Complete all required documentation and charting in a timely manner.
- Complete MDS assessments, care plans, and other required paperwork as assigned.
- Help oversee and direct resident care when needed.
- Maintain resident privacy and follow all HIPAA and company confidentiality guidelines.
- Must have an active and unrestricted NYS Registered Nurse (RN) license.
- Previous experience working as an Admissions Nurse in a long-term care setting.
- Strong clinical assessment and critical-thinking skills.
- Able to make sound decisions and work independently.
- Strong leadership and supervisory skills.
- Comfortable using computers and Microsoft Office programs, including Word, Excel, and Outlook.
- Long-term care experience is preferred.
- Strong communication and people skills.
Salary : $100,000 - $107,000
