This is a fully remote position where you will monitor patients' vital signs, identify clinical trends, and collaborate with providers to improve patient outcomes.

We are seeking compassionate, detail-oriented Registered Nurses (RNs) or Licensed Practical Nurses (LPNs) to join our Remote Patient Monitoring (RPM) team. This is a fully remote position where you will monitor patients' vital signs, identify clinical trends, and collaborate with providers to improve patient outcomes.
Responsibilities:
- Monitor daily patient vital signs through our Remote Patient Monitoring platform.
- Review blood pressure, pulse, oxygen saturation, weight, blood glucose, temperature, and other biometric data.
- Identify abnormal trends and escalate clinically significant findings to the appropriate provider.
- Communicate with patients by phone to assess symptoms, provide education, and encourage adherence to care plans.
- Document all patient interactions and clinical interventions accurately in the electronic medical record.
- Collaborate with nurse practitioners, physicians, and the clinical team to ensure timely follow-up.
- Meet Medicare RPM documentation requirements while delivering high-quality patient care.
- Active RN or LPN license in good standing.
- Strong assessment and critical thinking skills.
- Excellent verbal and written communication skills.
- Comfortable using electronic medical records and remote monitoring technology.
- Ability to work independently while managing multiple patients efficiently.
