Summary: 

Community Outreach Workers (CHW) come from the communities they serve, building trust and vital relationships. These trusting relationships enable the CHW to have effective links between their own communities/systems of care and can significantly lower health disparities. CHW’s provide access to services, improve the quality and cultural competence of care, create an effective system of chronic disease management, and increase the health knowledge and self-sufficiency of underserved populations. 

 

This position provides community outreach coordination to bridge the gap between communities and the health and social service system. By aiding communication between the provider and patient, educate community members how to use the health care system and establish better communication processes. CHW’s work with patients on everything from insurance assistance, translation and interpretation, transportation needs and assisting patients with referrals and health care navigation. This role may have an individual specialty such as:  Women & Children, Adults & Seniors, Chronic Disease Management, etc.  

 

Essential Functions: 

Bridge the gap between communities and the health and social service systems: 

  • Improve quality of care by aiding communication between provider and patient to clarify instructions. 

  • Educate community members about how to use the health care and social service systems. 

  • Educate the health and social service systems about community needs and perspectives.

Navigate the health and human services system: 

  • Make referrals and coordinate services and connect people needing services to said resources. 

  • Teach people the knowledge and skills needed to obtain care. 

  • Facilitate continuity of care by providing follow-up. 

  • Assist with paperwork (e.g., help with application for public assistance).

  • Advocate for individual and community needs: 

  • Articulate and represent needs of community and individuals to others. 

  • Be a spokesperson for clients when they are unable to speak for themselves. 

  • Involve participants in self and community advocacy. 

  •  

    Provide Direct Services: 

  • Promote wellness by providing culturally appropriate health information to clients and providers. 

  • Educate clients on disease prevention. 

  • Assist clients in managing their chronic illness. 

  • Establish health programs that address health issues and improve patient outcomes. 

  • Provide individual social and health care support. 

  • Organize and/or facilitate support groups. 

  • Refer and link to preventive services through health screenings and healthcare information.

    Build Individual and Community Capacity: 

  • Participate in outreach events to educate the community about services provided. 

  • Build individual capacity to achieve wellness. 

  • Build community capacity by addressing social determinants of health. 

  • Identify individual and community needs. 

  • Mentor other CHWs – capacity building. 

  • Seek professional development (continuing education).

    Minimum Qualifications: (Experience/Skills/Abilities/Education/Certifications) 

  • High School Graduate or GED. 

  • Bilingual candidates preferred. 

  • Community Health Worker certification (CCHW) or desire to achieve certification. 

  • Active member in community and knowledgeable about community needs. 

  • 2 years previous outreach, community health work, and customer service experience in a community clinic setting. 

  • Present professionally in the community 

  • Ability to utilize positive customer service skills. 

  • Ability to communicate clearly and concisely. 

  • Ability to identify problems and recommend solutions. 

  • Ability to maintain confidentiality. 

  • Organizational skills including careful documentation and attention to details. 

  • Maintain an orderly and a neat work area. 

  • Knowledge of care coordination and services available in the community. 

  • Knowledge of local state programs/funding. 

  • Knowledge of regulations and laws regarding appropriate use of patient health information. 

  • Knowledge of medical terminology. 

Physical Demands: 

While performing the duties of this job, the employee is regularly required to stand; walk, often for prolonged periods; use hands to finger, handle, or feel; reach with hands and arms; stoop, kneel, crouch, or crawl; and see, speak, and hear. The employee must regularly lift and/or move up to 10 pounds and frequently lift and/or move

Qualifications:
