Program Coordinator - Supported Living Services
Summary of the Role:
The Program Coordinator is responsible for overseeing the operation of the Supported Living Services (SLS) Support Team and ensuring high-quality service delivery to Consumers. This includes supervising Case Managers, Field Supervisors, and Support Staff, maintaining compliance with Regional Center, DDS, and company guidelines, and providing direct support when needed.
Program Coordinators are expected to begin their workday at the main office unless prior approval is given by the Director. All supervisors are required to participate in an on-call rotation, one weekend per month.
Key Responsibilities:
1. Supervision and Administration
- Oversee, guide, and evaluate Case Managers, Field Supervisors, and Direct Support Staff to ensure consumer files are in compliance with DDS, Regional Center, and company guidelines
- Oversee completion and maintenance of consumer files
- Ensure completion of assessments, Quarterly Reports, and ISPs in a timely manner
- Conduct regular and unannounced site visits to observe and support team performance; provide recommendations that align with company policies
2. Documentation and Reporting
- Monitor and approve clock-in/out accuracy, overtime, time-off requests, and other attendance-related issues
- Review staff schedules, service documentation, and service hours for accuracy and compliance
- Prepare and submit Special Incident Reports (SIRs) promptly and oversee documentation processes to ensure consistency
3. Emergency Response
- Respond promptly to field emergencies, following SIR and emergency protocols; report findings to Director
- Provide coverage and support in the absence of other team members
4. Training and Development
- Provide ongoing training in accordance with the program design and individual service plans (following approval of the Director)
- Facilitate professional development opportunities at least quarterly
- Conduct monthly team meetings to ensure consistent communication and growth with your team
5. Performance Management
- Address staff performance concerns and implement improvement plans as appropriate
- Submit all performance concerns in writing to Human Resources and Director immediately
6. Operational Oversight
- Collaborate with HR and management to ensure procedures, communication materials, and policies remain current and compliant
- Model professional behavior and integrate best practices into daily operations
- Report field efforts, observations, and outcomes to Director or designated leadership
Qualifications:
- Education: High School Diploma or GED required
- Experience: Minimum of 3 years of relevant experience (training will be provided)
- Skills: Strong communication, time management, ability to coach and teach independent living skills, knowledge of local resources
- Certifications: Valid CPR/First Aid (must obtain within 90 days of hire)
- Physical Requirements: Must be able to lift up to 50 lbs.; must pass a physical and TB test
- Compliance: Must pass a criminal background check; must have valid vehicle insurance and registration

- Maintain professionalism in all interactions with consumers, families, partners, and all other agencies
- Respond promptly to all communications, including those involving emergencies and general inquiries

Pay: $23.33 - $28.10 per hour
Expected hours: 35 – 45 per week
