Centered Care is partnering with Bickford Senior Living to deliver personalized, whole-person care on-site inside Bickford communities. We are hiring a Health & Wellness Coordinator – Care Concierge who thrives in a hands-on, resident-facing environment and wants to make an impact where care actually happens—on location, every day.
About the Role
This is a fully on-site role located within assigned Bickford branches Bickford of Suffolk (3 days/ week) with additional hours as a Wellness/ Floor Nurse or Memory Care Oversight. The Care Concierge works side-by-side with residents, families, caregivers, and Bickford leadership to support chronic care management, dementia care navigation, and integrative behavioral health services. This role is embedded in the community
You will split your time on-site at the Bickford community between:
- Community health & wellness support (on-site, resident-facing) floor work
- Care Concierge clinical coordination and behavioral health programming (also on-site)
What You’ll Do (On-Site, In-Community)
- Build and manage personalized care plans for residents within the Bickford branch
- Coordinate care with physicians, providers, and families while physically present in the community
- Provide dementia care navigation and behavioral health coaching on-site
- Support care transitions (hospital, SNF, rehab, back to Bickford)
- Proactive care management and meeting resident and family unspoken needs
- Utilize Centered Care CareFlows technology to review resident risks and maintain strong clinical quality programs
- Partner daily with Bickford Health & Wellness leadership
- Deliver health education, wellness programming, and care coordination inside the community
- Serve as a visible, trusted clinical presence for residents and families
Who You Are
- Clinical nurse background with experience in senior living, chronic care, dementia, or behavioral health (LPN, CNA, RN)
- Experienced in senior living communities
- Comfortable working on-site in an assisted living environment
- Strong communicator who values face-to-face collaboration
- Organized, proactive, and relationship-driven
- Passionate about helping seniors remain independent as possible longer

32 - 40 hours salaried
Bickford of Suffolk - 3 days HWC / 1-2 days Floor Nurse and/or other Community Role
Pay: $33.00 - $38.00 per hour
