GENERAL SUMMARY
The Case Manager provides direct case management support to program residents, maintains accurate records, and assists the Program Director in delivering services in accordance with program policies and state regulations.
KEY RESPONSIBILITIES
- Provide case management services to young parents and families, including intake, assessment, service planning, and ongoing support.
- Coordinate client intakes, including communication, documentation, and transportation arrangements.
- Complete required assessments, safety plans, treatment plans, and documentation within established timeframes and regulatory standards (DCF, EEC, and program guidelines).
- Maintain accurate, organized client records and ensure compliance with documentation systems (e.g., Virtual Gateway, Advocate Notes).
- Facilitate and participate in treatment team meetings, case reviews, and care coordination with external partners (DCF, DTA, schools, and providers).
- Monitor client progress, update plans as required, and provide regular status reports to stakeholders.
- Support residents in achieving goals related to parenting, education, employment, housing, and independent living skills.
- Ensure residents’ basic needs and living standards are maintained (health, hygiene, safety, and environment).
- Provide leadership, guidance, and oversight to staff on documentation and client care practices.
- Coordinate appointments, referrals, and community resources, including healthcare, childcare, and housing services.
- Participate in on-call rotation and program operations, including meetings, trainings, and quality improvement initiatives.
- Assist with program administration tasks such as billing support, fee tracking, and reporting.
- Conduct weekly individual meetings with residents and facilitate group programming.
- Perform additional duties as assigned.
QUALIFICATIONS
- Bachelor’s degree in Psychology or a related field required.
- Minimum of two years’ experience in a residential or human services setting.
- Ability to work flexible hours to meet program and client needs.
- Strong communication, organization, and time management skills.
- Demonstrated ability to use collaborative, trauma-informed approaches to behavior management.
- Commitment to diversity, equity, and inclusion, with willingness to participate in related trainings and initiatives.
- Ability to successfully complete initial and ongoing competency evaluations and professional development requirements.
REQUIRED TRAININGS & SKILLS (PROVIDED BY CENTERBOARD)
- CPR/First Aid certification.
- Crisis intervention and physical restraint training.
- Medication Administration Program (MAP) certification.
- Training in HIPAA, OSHA, ethics, professional boundaries, and trauma-informed care.
- Participation in orientation and ongoing required trainings, including DEI-related learning opportunities.
- Maintain all required certifications and complete periodic recertification trainings.
